*The funeral home will need the following items for the person these arrangements are for.

     •Photo ID – forms acceptable: U. S. driver’s license, state-issued non-driver ID, Passport

     •Proof of Social Security - forms acceptable: actual card, tax form (W-2 or 1099), military record

Family Information

*The funeral home will need the following item(s) for the person these arrangements are for.

     •Military Discharge document (DD214)
